Transitional Care Program

VNA Community Healthcare's Transition Care Department helps patients stay in control of their health by coordinating a safe and positive transition from a health care treatment setting to home.  A full description of our home health care services is attached below.  Call the Transition Care Department at 1.866.862.0888 or 203.458.4275 with questions.

Patients hospitalized at Yale New Haven Hospital or the Hospital of St. Raphael, who choose to use VNA Community Healthcare for home care, will receive a pre-discharge visit or call from a Transitional Care Nurse to explain how our services work.

In June 2010 VNA Community Healthcare acquired VNA Services, Inc. from the Saint Raphael Healthcare System. The combined non-profit home healthcare agency operates under the name VNA Community Healthcare and provides home care services to residents of the Greater New Haven and Middlesex counties. The main corporate office is in Guilford, with a clinical office in Hamden.

During the past five years, VNA Community Healthcare took care of my mother-in-law who had both dementia and multiple medical problems. Their Transition Care Dept. smoothed the multiple transitions from her hospital stays. The visiting nurses and physical therapists helped to keep her active until almost the end of her life. They visited her in, first, her home and then in an assisted living unit. Their caring staff made such a difference for our entire family. I highly recommend this non-profit agency.
