Become a Health Care Advocate

December 5, 2013

A free 32-page booklet on how to become a patient advocate is available on the Web site of VNA Community Healthcare or by calling 866.474.5230 (toll-free in CT).  Additional tips and guidance attached below.

VNA Community Healthcare is a central CT provider of visiting nursing services, family caregiver support and home aides. We connect the Connecticut community with geriatric care management, support for recovering at home after surgery, and chronic care at home. We offer guidance about Medicare and Medicaid homecare and a calm voice to discuss healthcare options in CT.
