We’re Joining Forces
We are pleased to announce that on June 7, 2010 VNA Services, Inc., a nonprofit visiting nurse association formerly owned by the Hospital of Saint Raphael and based in Hamden and Madison, was acquired by VNA Community Healthcare (VNACHC). Most VNA Services staff have joined our team.
 

Some Things Will Never Change
These include our stronger-than ever commitment to our Beside You at Every Turn philosophy, a human voice at the end of the phone, minimal red tape, a can-do attitude, lots of caring and, of course, our community programs.

A Few Things Do Change

• VNACHC and affiliates will now employ 585 clinical and administrative staff and will serve about 1,800 patients daily. 
• VNACHC will keep its main corporate office in Guilford and will have a clinical office in Hamden.
• Our service area will expand to include the Greater New Haven area, and the shoreline to Old Lyme. Click on PDF below to see a map of the Towns We Serve.
• We will accept Medicare, Medicaid and most insurances, including Healthnet.
• We will now become the “Town VNA” for Hamden, North Haven, Madison and Woodbridge.
• Former VNA Services, Inc. patients’ will have access to new services: psychiatric nursing, a family caregiver support network, wellness programs and a VNA Helpline for assistance with finding services.
• Our affiliate, LifeTime Solutions will offer private pay homemakers, personal care assistants, live-in aides, Link to Life emergency response and medication management throughout our newly expanded service area.

We Are Taking New Patients
The VNA Community Healthcare Transitional Care Department continues to take new referrals from physicians, hospitals, rehab facilities, community agencies or patients and families themselves. To make a home care referral call: 1.866.862.0888 or 203.458.4275.

Making the Transition to Home Easier
Patients hospitalized at Yale New Haven Hospital or the Hospital of St. Raphael, who choose to use VNA Community Healthcare for home care, will receive a pre-discharge visit or call from a Transitional Care Nurse to explain how our services work.
 

Strong House AdAdult Day Center Becomes an Affiliate
In addition to the purchase of VNA Services, Inc. VNACHC has also acquired Strong House Adult Day Center in Madison. The program provides weekday recreation, socialization, good nutrition, nursing care and transportation for older adults with dementia or other health issues. Strong House is accepting new clients.
